What to Bring and Wear

What to Bring

Please bring the following items to your first visit:

    • Any relevant medical documents if available, such as PT prescriptions, X-rays, MRI reports, or a medication list. You do not need a PT prescription to begin your first physical therapy session in New York. However, if you already have one, please bring it with you.

    • A photo ID (required to enter our building)

What to Wear

Please wear comfortable clothing such as a T shirt and athletic shorts or pants.

You may also change clothes at our office if needed.

What to Expect at Your First Visit

At our Midtown NYC physical therapy clinic, your first visit focuses on a
thorough evaluation and personalized care planning.

Check-In & Documentation

When you arrive at our office, you will register at the front desk. We will need your insurance information and ID card, as well as any other paperwork you have brought with you, such as a referral, X ray or MRI reports, and other medical records. If you have not completed your online intake forms in advance, you will be able to complete them in the office before your evaluation. Following that, you will be evaluated by one of our physical therapists. evaluated by one of our physical therapists.

Comprehensive Physical Therapy Evaluation


Your evaluation begins with a detailed review of your medical history, injury
history, current symptoms, and activity level. Your physical therapist will ask
specific questions about how and when your symptoms started, what movements aggravate or relieve your pain, and how your condition affects your daily activities or athletic performance. This clinical interview helps identify potential contributing factors and guides the physical examination and
treatment planning that follow.

Physical Examination and Treatment Planning

During your evaluation, your physical therapist will assess your range of motion, strength, posture, and movement patterns. Hands-on palpation is also used to evaluate tissue quality, including areas of muscle tightness, trigger points, or scar tissue that may be contributing to pain or limited function. When appropriate, specific clinical tests may be performed to help identify the source of your symptoms. Based on these findings, your therapist will develop a personalized treatment plan, and your first visit will include some treatments to alleviate pain and begin the recovery process.

Personalized Treatment & Ongoing Care


Each follow up session will build on your initial evaluation and treatment plan, with ongoing adjustments based on your progress and response to therapy.
Your physical therapist will regularly reassess your movement, symptoms, and functional abilities to ensure that your care remains effective and aligned with your recovery goals.

 

Your therapist will also discuss your short term and long term goals and, based on clinical findings and professional judgment, will recommend how often and how long you should attend physical therapy.
Before leaving your initial appointment, you will schedule your next sessions at the front desk to support consistent progress and continuity of care.
